2011 - 2012  Soccer Registration

Please complete a registration form and you will also need to provide a copy of your child’s birth certificate if they are new to WYSA.  The spring season is a continuation of the fall season, therefore, if you registered last fall you will not need to re-register.

All soccer players must have a signed medical release form and a copy of the players birth certificate on file before practicing or playing soccer games. If you have not already done so, please provide copies to the Executive Director prior to your first practice or game. Download Medical Release Form

In addition, all players must participate in a cookie dough fundraiser or pay the $25 buyout instead. If you are registering more than one child in a family, the fundraiser buyout is reduced to $15 for each additional child after the first one in the family. Download cookie dough sheet

If you need financial assistance to pay registration fees, contact the WYSA Executive Director for information. WYSA can utilize a portion of the funds it receives from United Way to offset all or a portion of your registration fees in cases of financial need. Download Scholarship Form

Petitions to Play Up or Down: Parents can petition to have their players play up one division or down one division, but those petitions will only be approved if there are good reasons that are supported by facts and/or recommendations of former coaches.
